RE: Letter of Intent

Dear Foundation Administrator,

The Queens Medical Professionals Association (QMPA) is a non-profit organization that has worked tirelessly to improve patient health outcomes in primary care practices throughout Queens County. Originally founded during the computer technological revolution, we first focused on the implementation of Electronic Health Record systems (EHR). We have trained over 500 health care professionals on how to use EHR software. This newfound knowledge has paved way for new metrics of health outcomes in patient care. EHRs are able to supplement qualitative data with quantitative data for analysis of core measures.

In Queens County alone there are over 5,000 physicians and 1,500 physician assistants caring for 2,333,054 people. One of QMPA’s main service areas, Corona, is also registered as a medically underserved population. QMPA seeks to alleviate conditions by training physicians and providers on site adjustments to improve patient health. A series of weekly training sessions about EHRs in conjunction with discussion about new health measures will be held on Wednesdays and Saturdays. In addition, each practice will be assigned a Project Officer on site. The officer will explain possible models and structure milestones. The models will lead the practice to experiment with new workflows. Quarterly performance will be monitored and reminders will be given for unmet measures. The first test group for will be focused in Elmhurst, Queens and for the goal of supporting better health outcomes for residents of Queens.

For this transition assistance project we request $900,000 from the Robert Wood Johnson Foundation. The funds will go towards office management, consultant fees, quality assurance, and performance evaluation. We expect at least $10,000 of in kind donations from our patrons to cover miscellaneous expenses. Thank you.
